Report of Funded Projects

Past funded projects

2005-2006

Robert Wood Johnson Foundation Positive Deviance MRSA Prevention Partnership: Use the method called Positive Deviance to redesign care to reduce MRSA infections.

Role: Co-Investigator

2009-2013

Reduce MRSA, Centers for Disease Control and Prevention (CDC) Prevention Epicenters Program and the Agency for Healthcare Research and Quality (AHRQ) A three-arm randomized cluster trial to determine best intervention to reduce infections in the ICU. This involved 43 hospitals and 74 ICUs.

Role: Co-Investigator

2013-2015

Optimizing Pre-Operative Antibiotic Prophylaxis in Cardiac and Orthopedic Procedures. AHRQ contract # HHSA2902006100021I. Develop a best practice algorithm to reduce complex infections in orthopedic and cardiac surgery across 20 hospitals

Role: Co-Investigator

2010-2012

CDC/IHI Antimicrobial Stewardship Project, Public Health Foundation. Goal to develop a driver diagram outlying factors necessary to optimize antimicrobial use and improve outcomes while reducing adverse events. This was an 8-hospital project and measurements were developed to determine impact

Role: PI

 2016-2017

Estimating the national burden of sepsis using electronic health record data, NIH / CDC 3U54CK000172-05S1. The goal of this project is to utilize electronic health record (EHR) data from a large network of diverse hospitals to estimate trends in sepsis burden using the surveillance definition that we have developed and validated.

Role: Co-Investigator

Current funded projects

2013-

ABATE, NIH ABATE Infection Trial 1 UH2 AT007769-01. Goals to determine is CHG bathing in non-critical care reducing healthcare-associated infections.  This is a 53 hospital in over 190 units in a 2 arm randomized cluster trial. Trial complete data being analyzed.

Role: Co-Investigator

2015-

Swap Out, CDC 2016-2020 1U54 CK000484-01 Epicenter IV: Harvard Pilgrim-UCI Center for HAI Prevention. Swap Out Trial: 2 arm randomized trial in adult ICUs 140 hospitals: arm 1 mupirocin intranasal plus CHG bathing; arm 2 iodophor intranasal with CHG bathing. Key outcomes S aureus clinical cultures, all cause bacteremia, and emergence of resistance to mupirocin and iodophor

Role: Co-Investigator

2016- T

he CLUSTER Trial: Cluster Linkage Using Statistics to Trigger and Evaluate Response. CDC 2016-2020 1U54 CK000484 Epicenter IV Large Multicenter. Project. This supplemental grant is to conduct a cluster randomized trial of 80 hospitals to reduce the size and duration of hospital outbreaks using a space-time statistical software tool for alerting clusters

Role: Co-Investigator

2016-

INtelligent Stewardship Prompts to Improve Real-time Empiric Antibiotic Selection for Patients - The INSPIRE-ASP Trial. CDC  SHEPheRD Task Order 200-2011-42037 TO 2016-003. The goal of this project is to conduct a cluster randomized trial of 40 hospitals with the goal of reducing empiric extended spectrum antibiotics in non-critically ill adults hospitalized with pneumonia and urinary tract infection. The trial compares routine care to a point-of-care smart tool providing real-time probabilities of MDRO infection in this population

Role: Co-Investigator

2016-

Detection and Response CRE and C. difficile. CDC Grant Epidemiological and Laboratory Capacity (ELC), K1 (Focus on Detections and Response), K2 (Focus on Coordinated Prevention and Stewardship). Goal to collect CRE and C difficile isolates across great Houston to perform whole genome sequencing and other molecular studies for tracking resistance and infections across the continuum of care. Stewardship opportunities will be identified along with suggested interventions

Role: Co-Investigator

2018-

Surveillance of Multidrug-Resistant Emerging Healthcare Pathogens, Texas Medical Center Health Policy Institute grant

  1. To establish a real-time surveillance system for Candida species isolated from hospitalized patients with bloodstream infection from at least 20 participating hospitals in the Texas Medical Center and surrounding Houston hospitals

  2. To create a reproducible, high-throughput analysis platform for whole genome sequencing of Candida species

  3. Disseminate results of the whole genome surveillance system to local, state, and national thought leaders

Role: Co-PI

INtelligent Stewardship Prompts to Improve Real-time Empiric Antibiotic Selection for Patients - The INSPIRE-ASP Trial

The goal of this project is to conduct a cluster randomized trial of 40 hospitals with the goal of reducing empiric extended spectrum antibiotics in non-critically ill adults hospitalized with pneumonia and urinary tract infection. The trial compares routine care to a point-of-care smart tool providing real-time probabilities of MDRO infection in this population.  This is using real-time precision medicine to improve outcomes which is innovative.
